Yes, I know about when a man and a lady love each other very much, etc., etc., but weren't she and Brine taking any "precautions"? Did she mislead Brine about contraception and try to trap him with a baby?
Brian was initially shocked by the pregnancy and assumed or suggested that Shove would have an abortion. The clear impression was that Brian had not been buying anything for the weekend and that Siobhan was responsible for the contraception, though we don't know what method she was (allegedly) using. She told Brian that 'accidents happen'. Siobhan had suffered a miscarriage earlier and my view is that this was a deliberate pregnancy (or at least a Freudian no such thing as an accident)
When she told Brian, he said, "But I thought we were so careful?" This listener assumed (and it seems from Siobhán's reply, "These things happen!" )that there was a condom failre.
If all the 'condom failures' were real, genunine failures, ie. a fault in the manufacture rather than misuse of the condom, the manufacturers would be sued out of business. Maybe Siobhan, as a good Catholic, told Brian she was using the rhythm method. Brian, not being of this faith did not understand, but did not choose to enquire further. Hence the little offspring. The rhythm method lacks both reliability and spontaneity.

The "we were so careful" thing made me wonder if they'd tried the very dodgy last-minute withdrawal method. In any event, I suppose it's all too much for an early evening radio audience to cope with over dinner!
