The son of a woman murdered by a mentally ill man says that there needs to be major changes in mental health services to make sure dangerous people like his mother's killer are not released back into the community. 79 year old Eleftheria Demetriou was stabbed repeatedly by a man who'd been released from a mental health hospital just weeks before. Hakim Abdillahee had been treated for a personality disorder and was deemed "gentle and calm", but after he was released, he stopped taking his medicine and started using cannabis.

Also the health regulator for England, the Care Quality Commission, is to reconsider its decision not to publish the names of former senior managers identified in a report saying there was evidence of a cover-up at the organisation. Victoria speaks to the Information Commissioner, Christopher Graham, and asks whether redacting information on data protection grounds stand up?
