Teenager's life-saving transplant
Fourteen-year-old Chance has a rare condition which has badly damaged his heart. He is flown 200 miles for a life-saving heart transplant from Leeds to London's Great Ormond Street Hospital. There is just a five hour window to get Chance to the hospital and the Embrace team step in to get him there in good time.
