Health service 'should learn from mistakes'
Times columnist Matthew Syed claims politicians often refuse to admit they have got something wrong and that means they are not learning from their mistakes.
In a personal film for the Daily Politics, he took to the skies to look compare how the aviation industry has learned from their errors, but health providers are too scared of litigation and reputation issues.
He claimed: "That's why preventable medical error is one of the biggest killers in the UK - it kills way more people than traffic accidents."
Mr Syed will debate this film on Wednesday's Daily Politics around 12:40 GMT with MPs Brandon Lewis and Jon Ashworth.
